Breast Implant types

As a breast augmentation surgeon, Dr. Vladimir Grigoryants, uses two types of breast implants. These are silicone breast implants and saline breast implants. You have likely heard of silicone breast implants, as they have been the most popular type in recent years. Made of semi-solid silicone gel, this breast implant is popular for its natural feel and almost complete inability to rupture. The other kind is saline, which has a silicone shell but is filled with a saline solution. The purpose of this implant is a minimally invasive procedure, in which the silicone shell is inserted through a small incision, and the implant post-operatively filled with saline solution.

The type of breast implant you choose depends on a variety of circumstances, and these will be discussed at length with Dr. Vladimir Grigoryants, allowing you to make an educated decision.

The Two Major Types of Implants: Saline vs. Silicone

Saline implants are a silicone shell with a saline filler. They have a less natural feel than silicone implants but are generally less expensive and require a shorter incision, as they are filled after placement.

Silicone implants have a silicone shell and a cohesive silicone gel within. They require a slightly larger incision than the saline implants during the breast augmentation procedure. Silicone implants also come in two different shapes, round or teardrop. Round silicone implants look natural from any angle whereas the teardrop shape appears most natural in an upright position. The teardrop shape may also have potential to shift and require adjustment where if the round implant shifts, it does not alter the outward appearance of the breast.

Ideal Candidates for Breast Augmentation

Ideal candidates for breast augmentation want to increase the size of their breasts and have realistic expectations about the procedure, recovery time, and results. You must be over 18 years of age for saline implants or 22 years of age for silicone implants. If you are in good physical and emotional health and are not currently pregnant or intending on becoming pregnant in the near future, then you are probably a candidate for a breast augmentation. Ideal candidates are non-smokers who maintain a stable, healthy weight.

Breast Implant Placement

During your breast augmentation in Los Angeles, a small incision is placed under the breast, around the areola, or in the armpit. The placement of this decision depends upon a number of things, and it will be discussed with you during your initial consultation. Once the incision is made, a pocket is created underneath the muscle that is behind the breasts, and the implant is placed in that pocket.

Gentle Placement of Breast Implants

Dr. Grigoryants is very gentle when creating the pocket for the breast implant in order to cause as little trauma as possible to delicate breast tissues. This makes recovery much easier as gentle handling means there is less bruising and swelling. The size of the incision is kept to a minimum to ensure patient comfort as well as to improve the aesthetic result. A small incision leaves an almost invisible scar.

One of the benefits of saline implants is that the size of the incision can be smaller as the implant is not filled until it is in place within the pocket. A large implant can be inserted through an incision less than 2 inches long. Also since the implant is inserted empty, the size can be checked and adjusted as needed to achieve the desired size.

Silicone Implant Placement

Recently a device called the Keller Funnel™ has gained much popularity within the plastic surgery world. It allows a silicone breast implant to be placed in a substantially smaller incision using the same principle that bakers use to push icing through a piping bag.

After the breast implant pocket is made the Keller Funnel is used to transfer the implant to the pocket. The device is a made of a sterile surgical grade plastic with a wide top which tapers to the tip. Keeping in mind the size of the implant, the tip is cut using pre-marked guides. The implant is then placed in the device and the tip inserted into the breast pocket. Using a squeezing motion, the implant is gently pushed through the narrow opening where it expands into the pocket.

Reducing Breast Implant Complications

Breast augmentation using this device is much easier on the patient in several ways; first the incision is very small compared to what would be needed to place the silicone implant without the Keller Funnel.

Secondly, the device is sterile and allows the implant to be placed without ever touching the patient’s skin. This reduces the possibility of introducing bacteria into the breast pocket which then reduces the incidence of a complication called capsular contracture. Finally using the Keller Funnel markedly reduces the time it takes to place the implant which reduces overall surgical time.

Breast Implant Recovery

Because of this, discomfort following your surgery is at an all-time low. Any initial discomfort can be controlled using pain medications provided by our office. Additionally, you will wear a support bra for one to two weeks following the surgery, helping your breasts to maintain their improved shape and elevation while they recover.

We use absorbable sutures that do not need to be removed, saving you the hassle from coming back for such a minor purpose. Most of your swelling will resolve within several weeks, and within about 4 weeks, you may start using your arms for more vigorous exercises. Within several months, the breasts will settle completely, giving them a soft and much-improved appearance.
