Goals

The goal of rhinoplasty is to produce a shapely nose without compromising the natural beauty of a person’s face. If this is done correctly, it will result in a face that looks more proportionate and balanced. In addition to cosmetic goals, rhinoplasty may also be used to correct the function of the nose. Occasionally a patient will have a nasal interior that is misshapen, as is the case with a deviated septum, which can contribute to difficulty breathing. Rhinoplasty may be used to effectively handle this condition.

Closed rhinoplasty

While many nose surgeons practice open rhinoplasty, Dr. Vladimir Grigoryants prefers closed rhinoplasty, also known as endonasal rhinoplasty. This technique utilizes incisions made inside the nose so there is no visible scar across the columella. It may sound unbelievable, but incisions inside the nose may be used to reduce the dorsal hump or reshape the nose tip. Further, because the nasal skin is not lifted, as is the case with open rhinoplasty, the results of closed rhinoplasty are easier to predict during the surgery itself. Additionally, the convalescence period is much accelerated with closed rhinoplasty.

In an open rhinoplasty, the incision is made across the columella, which is the bottom portion of your nose between the nostrils. The skin of the nose is then lifted, exposing the nasal cartilage. The way in which a scar on the columella heals is not always predictable and will often result in a visible protrusion and thickening of the skin.

Restoring function

There is no better feeling than the smooth, unrestricted passage of air through one’s nose. Occasionally this is blocked by a defect in the nostrils, which may only be corrected with nose surgery. In any patient complaining of nasal obstruction, Dr. Vladimir Grigoryants can open the nasal airways and restore the fully capacity of airflow for the patient. Several techniques may be utilized, depending on the situation at hand. Correction of nasal airway obstruction maybe covered by insurances companies.

  • Septoplasty. In the case of a deviated septum, surgery may be used to correct the septum placement, putting it right back in the middle where it belongs. This serves to even the airflow capacity of each nostril, making breathing easier for the patient.
  • Turbinate reduction. The turbinate is a long structure in the nose that is parallel to the passage of air. It consists of bone covered in soft tissue that may enlarge and shrink depending on circumstances. If this turbinate enlarges, it will hinder the passage of air through your nose. Turbinate reduction serves to reduce the size of the turbinate, restoring the smooth passage of air.
  • Spreader grafts. These are used to effectively reconstruct an internal nasal valve that may have deteriorated. Spreader grafts are made from a patient’s own septum cartilage and are placed on each side of the septum. These grafts are important to prevent midnasal collapse as well as restore airflow through the nose.
